This is done with a special medication called a \u201csclerosant.\u201d The medication is delivered directly into the problematic vein using a very fine needle.<\/p>\n<p>The drug comes into contact with the innermost layer of the vein\u2014the thin membrane we call the endothelium. This contact creates an intentional, controlled injury in the vein wall. The body immediately initiates a healing process. This process causes the vein walls to stick together and fuse permanently. At the end of the treatment, the problematic vein turns into a fibrous cord that can no longer carry blood. Over time, the body reabsorbs and eliminates this nonfunctional remnant.<\/p>\n<h2>Which Medications Are Used in Sclerotherapy?<\/h2>\n<p>The medications that achieve this closure are generally called \u201csclerosants.\u201d They are classified into different groups according to their mechanism of action. In modern practice, the agents most commonly used\u2014and recommended by European guidelines\u2014are those in the \u201cdetergent\u201d group. The main types are:<\/p>\n<ul>\n<li>Detergents (Polidocanol \u2014 POL)<\/li>\n<li>Detergents (Sodium Tetradecyl Sulfate \u2014 STS)<\/li>\n<li>Osmotic agents (Hypertonic Saline \u2014 concentrated salt solution)<\/li>\n<li>Osmotic agents (Glycerin)<\/li>\n<\/ul>\n<p>Foam treatment in particular is based on detergent-group agents (POL and STS).<\/p>\n<h2>What Is Foam Treatment and Why Is It Preferred Over Liquid?<\/h2>\n<p>Foam treatment is one of the most important advances in sclerotherapy. It is the transformation of the liquid sclerosant into \u201cmicro-foam\u201d by mixing it with air at a certain ratio (typically 1 part drug to 4 parts air). This is prepared within seconds using a simple technique called the \u201cTessari method,\u201d with two syringes and a three-way stopcock. Or, today, it can be prepared with devices such as Varixio.<\/p>\n<p>So why go to the trouble of turning a liquid into foam? The answer lies in the physical properties of the foam.<\/p>\n<p>When you inject a liquid drug into a vein, it immediately mixes with blood and becomes diluted. Its concentration falls and its effect decreases.<\/p>\n<p>Foam, however, is much denser and does not mix with blood. When injected, it pushes blood ahead of it like a piston and clears the vein of blood. In this way, the drug remains at full concentration, without dilution, and stays in contact with the vein wall for much longer. This multiplies the effectiveness of the treatment.<\/p>\n<h2>Is Foam Sclerotherapy Really More Effective Than Liquid Treatment?<\/h2>\n<p>Yes\u2014especially in certain vein types, the difference is very clear. Scientific studies and meta-analyses have demonstrated this difference unequivocally.<\/p>\n<ul>\n<li>In large veins (varices and axial trunks): Foam is significantly more effective than liquid. All scientific analyses show that the success rate of liquid remains quite low in large veins, while foam offers a very high success rate.<\/li>\n<li>In telangiectasias (fine spider veins): A clear superiority of foam has not been proven for these very small, net-like vessels.<\/li>\n<li>Fewer sessions: Because foam achieves a much higher success in the first session, completing treatment typically requires fewer sessions.<\/li>\n<li>Ultrasound visibility: This is one of the most important technical differences. Foam appears bright white on ultrasound. This allows the operator to see, in real time, where the foam goes inside the vein, which regions it fills, and whether it reaches the source of reflux.
